Hospice of Santa Barbara
Hospice of Santa Barbara (HSB) offers organizational Advance Care Planning Workshops for individuals to learn about and complete their advance care plans. We also provide trainings and support for community individuals and organizations to become trained in facilitating Advance Care Planning workshops to help guide families through the process of completing the MyCare advance directive. HSB is available to support individuals and families with advance care planning questions and needs. These services are also available in Spanish through our sponsored initiative--Mi Vida, Mi Voz.

Sansum Clinic:
Sansum Clinic offers Advance Directive workshops, free of charge and open to the community. In addition to the workshop, patients of Sansum Clinic may get individual help with Advance Care Planning as well as free notary service for their Advance Directives. Cottage Health:
Health care providers and community members are welcome to contact the Advance Care Planning Program at Cottage Health. The Advance Care Planning Team can help with the following:
- Answering questions about completing advance directives and Physician Orders for Life-Sustaining Treatment (POLST) forms, in addition to answering legal questions
- Submitting a completed advance directive and/or POLST to an individual’s electronic medical record at Cottage Health
- Providing current Cottage Health inpatients with individual advance care planning counseling at bedside during inpatient stay
- Referring community members to organizations which will help them complete their advance directives outside of the hospital
- Providing MyCare advance directives and MyCare informational brochures to health care providers, professionals, and community members in Santa Barbara County
- Presenting information about advance care planning to health care providers and professionals in Santa Barbara County

What We Do
The Alliance was formed with three main purposes in mind:
- to develop and provide opportunities to foster conversations about living and dying
- to cultivate a dialogue that reduces the sense of isolation felt in death and dying, and that will reduce the stress of loved ones
- to enhance the quality and coordination of end-of-life care services in the County.
Who is The Alliance for Living and Dying Well
Our alliance includes core supporters for and providers of compassionate end-of-life care including: Archstone Foundation, Cottage Health, Hospice of Santa Barbara, James S. Bower Foundation, Santa Barbara Foundation, Sarah House, St. Francis Foundation, and Visiting Nurse & Hospice Care.
